Early Voter & Absentee Ballot Application
Absentee Ballot
Absentee Ballot Applications may not be submitted more than 30 days before the election. Qualifications to vote by absentee ballot:
- Absence from county on election day
- Temporary or permanent illness or disability
- Duties related to primary care giver of one or more individuals who are ill or physically disabled
- Resident or patient of a Veterans Health Administration Hospital.
- Detention in jail/prison, awaiting trial, awaiting action by a grand jury, or in prison for conviction of a crime or offense which was not a felony

If the application requests the absentee ballot be mailed, the application must be received by the district clerk no later than 7 days before the election. Otherwise, the application may be personally delivered to the district clerk not later than the day before the election.
Absentee Ballot – Military Voter
Original application must be returned by mail or in person to:

Military ballot applications must be received by District Clerk no later than 5:00 p.m. on the on the 15th day before the election. Call the district offices for more information.
Early Mail Ballot Application
Early Mail Voter Applications may not be submitted more than 30 days before the election.

- Greater Johnstown School District Certified Staff Application (PDF) for administrators, teachers and teaching assistants.
- Greater Johnstown School District Certified Staff Application (Google Docs Format) for administrators, teachers and teaching assistants. This application is a Google Doc, which enables applicants to make a copy and type information into the document, on the computer. Please e-mail the completed document to the superintendent’s secretary.

Facilities Use
The Greater Johnstown School District is launching ML Schedules, a new online system for booking facilities in the district. This new platform replaces the paper Facilities Request Form.

All forms in the prekindergarten application packet must be completed and brought to the Johnstown High School Office, Attention Central Registrar, together with proof of residency. All parents must complete the residency form.
